The APEX Population Health/Disease Management Program (PDMP) is designed to work with health plans and/or multi-specialty medical groups to improve care for patients with difficult wounds or limb threatening conditions. APEX can develop a limb preservation center  within the group(s). In addition, case managers and APEX medical experts will work with in-network providers to ensure optimal prevention, identification, assessment, and interventions to avoid repeated emergency room visits, unnecessary hospitalizations, loss of work, and unnecessary amputations.
